Is there a way to have 2 forms on my blog and both of them with different fields?

Use this add-on:
https://github.com/cdillon/strong-testimonials-multiple-forms
Look for the green “Clone or download” button then “Download ZIP”. Install in WordPress via Plugins > Add New then the “Upload Plugin” button.

This was going to be a premium add-on but I plan to incorporate it into the core plugin soon so just ignore any license settings.
