Multiple choice, "Other" text box

Great plugin Mike – I can’t seem to find a way to have a multiple choice “other” selection trigger a text box to denote the other. Or just have a text box next to “Other” to grab the info.

My workaround is a separate text box field immediately following the multiple choice/radio button field.
