Multiple config forms in the admin interface not working/displaying correctly

    Running this plugin primarily under multisite 4.0 but also installed it under single site 4.0 no plugins with the same problem.

    Logged in as super admin – under WP Membership/membership levels if you click Add level or Edit level, the forms you get are horribly broken. Ther is no text in front of the pulldown boxes etc – looks nothing like your documented screenshots.
    Also seems to be in other places for example settings/tools. I can provide screenshots if necessary.

    This is happening consistently in latest IE/Firefox/Chrome. Upgrading to 1.8.3 didnt help either (by the way the one on ww.wp.xz.cn is still 1.8.2 though it claims to be 1.8.3)
