Multiple Page Form Plugin

Hi!
I am searching for the following Plugin:

I want to receive Feedback from my guests and so I search for a Form or quiz-Plugin that shows each question on a new page (different from contact form 7).
You answer question 1 – next page. You type in your e-mail-address – next page.

And perfectly I would receive all the answers via e-mail (different from WP-Pro-Quiz).
Perfectly II I would be able to change the Answers, give them a code (for example: yes would be a “1” in the e-mail) to use them in SPSS.
Perfectly III: the answer would be sent as a CSV-attachment.

I know, wishful thinking.
