Multiple Reference

Hi, I really like this plugin, it does what it suppose to be.

I wonder if there a way to have more than one reference field.
So that people can leave both their name and email address if they want to. I tried having two “reference” in the shortcode, but it only show the latter one. I wonder if there another way of doing it.
