    I have determined some more information that may help pinpoint the issue. I am using the plugin Pro Sites by WPMUDev, and this message appears either when the user tries to login via the primary site (domain.com) (not their assigned subdomain xxxxxxx.domain.com), or when they try to upgrade their site using the Pro Sites mechanism for doing so. Pro sites redirects to http://domain.com/pro-site/?bid=N. Thanks again for the help.

    Hello,

    This is the expected behavior for multi-site configurations. Since WordPress allows users to sign into subdomains other than their own, Duo forces two-factor on these types of logins to prevent a sitaution where users could bypass two-factor by signing into a subdomain which does not have the two-factor plugin enabled.

    Looking at the way the Pro Sites plugins works, I think this plugin simply isn’t going to be compatible with Duo WordPress.
