After exporting, please share the file contents with us using the free services such as Google Drive, https://pastebin.com or https://justpaste.it
Please always make sure to use such services to share the code and don’t post the code here directly as it will most likely be unusable.

It appears you have shared an incorrect file, you shared the submission, please remove it immediately as it is a public forum.
What we are asking is form export, which would allow us to import the form you have created in order to test the form and see whats causing the issue.

If you are concerned about any sensitive information in the form, then you can duplicate your form, remove any sensitive information, and then export it.
You can share the export file via Google Drive, Dropbox or any cloud service in the next reply.
