Hello Pete are you using version 2.2.9? If so then first name and last name fields can be added from the user tab.
If not, then add fields with the name “first_name” and “last_name”.
Regarding the profile picture, that is a bit more complicated since the data is pulled in from gravatar. Can you use a custom field for the profile picture?

It’s possible to replace the gravatar with a custom avatar. Our plugin provides this option. Create an image field and then go into the general settings.
On the bottom you will see this option
