Need a SIMPLE Contact List builder.

I know there are many “contact builders” and list builders, but most have features that make them useless to me. I need a SIMPLE form with name and email, that will then add them to a list that can be downloaded. (CSV or whatever)
Some make you use a paid service, some make them go through a “double opt-in form”, etc.
Just need a simple form that will put their email in a list, then redirect to a page.
If you know of one, please let me know.
