Need Advanced Form Plugin, PayPal + Upload

My company needs a new form built for an upcoming award show that we put on every year for new products in the hearth, barbecue, and patio industries. We’re looking to make minimal work for both our contestants and for us (but mostly our contestants). Because of some of our specific needs, I’m really struggling to find a form that can do what we need it to do.

I am looking for a WordPress form plugin that can do the following:

• Allow me to use HTML/CSS to style fields as needed and also organize detailed instructions

• Allow multiple text fields in a single questions (we’re asking for indoor booth numbers and outdoor booth numbers for the show that we are hosting the award ceremony at in the same question – need two fields in the same question to transfer cleanly to Filemaker)

• Allow conditional logic so that contestants wanting to enter more than one product in a single form can do so while keeping the form appearing clean and short

• Use Paypal for transactions

• Allow uploads up to 200MB

Ideal plugins would also include:

• Scheduling the price per product to change after a certain date ($200 before January 15th, $250 after)

• Use the conditional logic to deduce the price of the entry ($200 per product entered – need the form to add an additional $200 every time the question “Enter Another Product?” is answered “Yes”) so that contestants only have to fill out the form once

I apologize if this is the wrong place to post this question, but I am running out of time to get this website up and running and this form is absolutely essential. I’ve been searching for a couple of days for a form plugin and I have contacted a couple of plugin support teams asking them my specific questions, but things are looking bleak, so I thought it wouldn’t hurt to ask the WordPress forums.

EDIT: We would be willing to pay for a form that meets the majority of our needs.
