NEED PLUGIN: User Submitted Files

I need a plugin that allows my users to submit a file, address a description, and submit for approval. I don’t want to force the situation from a “comment” paradigm, but from a clean NEW entry if possible. This is for a developer website where folks will be encouraged to contribute source code, and other related files.
