Network installation Form missing

    I’ve managed to create “network” under “Tool” for multi-sites.
    However, when I came to the page, “Create a Network of WordPress Sites”, I got the following message but the Form is missing! Why?

    “Welcome to the Network installation process!
    Fill in the information below and you’ll be on your way to creating a network of WordPress sites. We will create configuration files in the next step”

    There’s no Form for me to fill in the information! How do I overcome this problem?
