New plugin for a form

Hi,
I want to create a plugin using code.
This plugin facilitate me like this ..

I link will come below dashboard ,on clicking it a form will come which will contain name , contact no. and address field only.
On submitting data will be saved.
For this i tried some posts but not working .
which hook and file i should use please any body help me !
