No checkboxes showing

    Hi , really need your help. I have tried allot of teh fixes i found on here but nothing seems to work.
    The checkboxes dont show in teh contact form
    Instead all i get is

    Do you have? :
    .Test1.test23.test3

    Could you please help? thanks In advance
