No Custom Settings

I’ve inserted the widget into the content dock (scroll down the page at xtheme2.cabforward.com to see the content dock pop-up on the left hand side), but none of the custom settings are showing up. There’s no header or sub-header, the submit button text hasn’t been changed, and you can’t even complete the form becuase you can’t see the checkboxes. If I enter an email address it says “please select at least one list to subscribe to,” but I can’t check what I can’t see.
