We don’t work on the weekends, but I’m sorry for the trouble you’re having! If you delete and reinstall the plugin, do the database tables get created? If not, can I check this out on your site? In order to provide private login info, you’ll need to create a private topic in our help desk:

Actually, before providing login info, there are a couple more things you’ll need to check:
1. Check your MySQL permissions to make sure you have permission to create tables.
2. Try clicking the “Uninstall Formidable” link (on the Formidable Global Settings page). This will re-create the database tables on the site, but it will delete any Formidable forms or entries on the site as well.
