Can you post a link to a page that includes a “missing” image?
Have you tried:
– deactivating all plugins to see if this resolves the problem? If this works, re-activate the plugins one by one until you find the problematic plugin(s).
– switching to the default theme to rule out any theme-specific problems?
– checking your uploads path in Settings/Miscellaneous?

I have switched to the default theme with the same result.
The path in Settings/Miscellaneous is wp-content/uploads with the option to Organize my uploads into month- and year-based folders checked.
Should there be a specific path like: wp-content/uploads/2010/04 instead?
Here is the URL to a page with a missing image (sounds contradictory doesn’t it)

I only have one plugin which is Podpress and disabling it does not help.

No. The settings you have appear to be correct.
It looks like it might be a permissions or .htaccess issue. The image url on that page points to http://girirajswami.com/blog/wp-content/uploads/2010/04/03.Rohini_initiation.jpg
But when I tried to access the image directly, I got a 403 (Forbidden).
That path is correct so why the forbidden notice? What can i try next?
Have you checked your .htaccess files?
i contacted my service provider and changed the permissions on that folder to something recommended in one post (i can’t remember the number)

What chmod/permissions did you use? 755? Did you apply it to the uploads, 2010 and 04 folders?
Yes 755 applied to the whole wp-contents folder
Recursively? Have you checked the 04 folder?
