    Tom,

    I don’t see the tracking code being added. Could you please confirm if you’re using v2.0.3?

    Also, do you have the cache fix option turned on?

    And, lastly please try clearing the complete W3 Total Cache. If you’re logged out of the site, you should be able to view the source of the page created and check if you can find “activate_counter”
