I understand that you are using 2FA from our Defender, right? Or is it a different plugin/solution for 2FA but the issue happens if Defender is active (and only if it’s active)?
I’d also need to know a few more things:
– is there any other security plugin active other than Defender currently (and if yes, what is it)?
– is there any cache active (as plugin on site or a server-side cache or both) and if yes, what kind?
– what PHP version is powering your site
– and does the issue happen always or more “randomly”; and in all browsers or only some specific one?
Let us know please and we’ll then see what to do next to identify the cause and fix it.

I’m getting ths same error in several websites. I have to try two or three times before I can login. For one of my websites I wasn’t able to login; I tried several times, so I trashed out cookies but still not able to login, then I tried in private window and worked fine. I have the login mask, 2FA and recaptcha active. Defender is the only security plugin installed and I’m running PHP 7.4.
I supose its a problem related to cache system or something like this. I just don’t know if its about cache plugin, browser or CloudFlare cache system.
