This problem is related to TFA plugin. If TFA is activated, and you click “Log In” button, all standard fields are hidden. Instead of them, a user sees “One Time Password” field and “Log In” button which has no “name” attribute compared to the standard “Log In” button. That is why captcha accepts every answer.

Thanks for your reply. Please read before writing: I have NO TFA installed or activated. And as you didn’t provide a solution, I deleted Captcha.
