Only some usernames reported

I’m using this plugin to get feedback on courses and lessons in learndash. Users are always logged in to access the content, yet for about 50% of the ratings no username is included. Some have it, some don’t. Any insights as to how I can make them reliably appear? We reach out to users that are dissatisfied – but can’t do that if we don’t know who they are.
