Looking over your site I do see the main pop up working for me. It says “Do YOU want to Make $688 a Day Starting TODAY using this 1 FREE Trick?”. Is that the one to are referring to?
For the sidebar, can I get you to go back to Appearance –> Widgets and open the Sidebar Optin Form in the widget area and select your optin form from the dropdown menu. Sometimes people miss that part.

Your assistance was helpful but I realized I didn’t have my sidebar widget setup.
I setup my sidebar widget, then followed your suggestion and it helped. My problem now is the whole widget isn’t showing.
Is this because I have to shrink something to make it smaller?

Thanks for getting back to me about that.
Yes, I can see from your link that the optin form is too wide for the widget space.
To make the optin form skinnier, you can go back to Optin Cat and find that sidebar widget. Now click on the “Form” drop down on the right that will allow you to change the colors and at the bottom under the “Border Color” selector you will see the “Width” setting.
Perhaps you can adjust that width setting to around 220px and then save it and see if that now allows the optin form to fit completely into the sidebar space.
You may need to adjust the 220 up and down a little to get it just right.
