OptinSkin Plugin Title Issue

I’m having an issue with the title displaying on my subscribe box at the top of my blog sidebar.

I’m using the optinskin plugin to create the subscribe box.

This subscribe box worked well for a couple of weeks and then the title started showing up about half the time – “get updates (it’s free)”

I tried recreating the subscription box and the same thing is happening – the title only shows up when it wants to.

Any ideas?
