Options Missing from Google Auth Settings

One of my users got a new phone, and I went in to his settings to generate a new QR code. Under Google Authenticator Settings, the only options are “Hide settings from user” and “Active”. Any idea how to get the other options back? I have had to deactivate it for this account for now, but I need it back, as we are a big target for hackers.
