order + image add-ons + quantity

Hello, great plugin!
I have two little problems:
1 – I can’t upload image for add-ons, when I click on the grey square nothing happens
2 – When I have more add-ons I can’t change the order, the arrow-button doesn’t work.

what I’m wrong?

In the Premium version is there a feature to display quantity for every add-ons?
For example: I have a group called Chamrs, inside there are 3 checkbox: heart, star, cat.
I’d like to have 3 fields quantity, one for heart, one for star and one for cat.
