Order tracking multiple query

I have few point or query on “Order tracking” which I just purchased now.

1.>I wanted to have a fresh form for customer to fill , how to do that ?
2.>I should be able to search the order based on dropdown or checkbox selection selection I made on custom field ?
3.>Even when I chooses “customer Name” in option , the dropdown list of customer doesn’t come to front end form
4.>I cannot able to set a time zone for “Riyadh, Saudi Arabia”
5.>I’m going to install it on local intranet server which doesn’t have internet access, do I must have internet at least for license update or is there anything manually we can do ?
