Orders failing

Hi,
I keep getting order failed messages, and then sometimes the order ends up going through and switching to processing, sometimes not- Here’s the message I recieve when the order fails:

Order status changed from Pending payment to Failed.

10 minutes later i sometimes get a IPN Payment completed.
But not always.

I'm getting it both when customers pay with paypal or with square.
