The address is taken from the one saved to the order data. Do you have another plugin that handles shipping that may be adding or editing the shipping address?
I went back to see what else was using the invoice and packing slip, it turns out that woo shipping is also active. However, I deactivated woo shipping and am still having the same issue. When I try to modify your invoice and packing slip, there is no place to add or update billing and shipping fields. And now, invoices and packing slips are only showing the shipping address on both, whereas, before invoice only showed billing info and the packing slip-shipping.
Also the customer and shipping has no name header fields anymore.
Please advise.

The address data is taken from the order. So if you want to edit the address that appears on the invoice, you have to edit the address in the order itself first. This is the general way our plugin works – it does not manipulate order data itself, it only prints the data as saved to the order.
