No, No errors.
When redirected to the login page it shows it as a blank page apart from my menu bar and site logo.

I have to refresh the page to see the login box.
Can you change theme and disable other plugins to test again?

Hi there.
I’m faced with the same problem. I’ve installed the plugin – and the auto-generated pages appeared in my menu.
The forms just appear after a reload/refreshing the page.
Did you already solve that problem?
Even if it’s a problem with a theme – what will I have to do to get it work with that theme?
