    I set it up using these plugins:

    CPT UI << to create the CPT and Custom Taxonomy.
    Custom Post Type Permalinks << this to set the permalinks

    UPDATE:

    I had the CPT set to have a hierarchy! changed it to false! FIXED!

    That’s a tough one that I don’t have an answer for, really. It’s a lot of information being stored in a given URL, and if the permalinks are giving 404 messages, then WordPress isn’t managing to parse it out into what you’re intending.

    Without being able to see the request being made, I’m assuming that it’s thinking the face-surgery and rhinoplasty and before-and-after portions are all child posts in this post type, when they’re actually meant to be the custom taxonomy and i assume terms in the taxonomy. All mixed in with the post type and single post.
