Permalink issue

Hello, I have tried everything I have found to fix this permalink issue. I have 1 page (only) that is showing “sample page” and cannot seem to get rid of it. I am uncertain as to what is different about this page to all the rest. My permalink settings are on Postname and I have tried the others to no avail.
