Since the permalink doesn’t change after renaming a file, does this present any SEO issues like duplicate content or WP performance issues or redirects, etc.?

The permalink is for the “attachment” post type, which is best redirected to the image URL for SEO purposes.

Renaming the file obviously changes the URL of the image, though, and unless it’s redirected, it goes missing. However, if it’s embedded on a page, it’s found again as soon as the page is re-crawled, which should happen every few weeks, so that’s probably not a big issue.

Duplicate content is not an issue in either case, because the attachment post doesn’t change and the old URL of the image no longer works, so there’s only one of it.
