Permalinks problem–could it be Yoast?

I have been plagued with problems with my permalinks for quite a while now. They are constantly causing crawl errors. I’m hoping someone can help me. Here’s what the links are displaying:

http://onlineincomeandresources.com/how-to-find-a-niche-market/onlineincomeandresources.com/home/

For some reason the domain name displays twice and in the wrong place.
