[Plugin: Add Local Avatar] broken image from second comment

uploading no problem. but a strange thing is happening. image is showing in the profile pic of the user and also in his first comment. but from the second comment the image breaks. i have already tried solution suggested by viviannie but it did not work. i repeat, image uploading no problem.
