Hi hector,
You need to get your API key, otherwise the map will not work.
Read the documentation of the plugin for more info.

Also, check your theme because the javascript files essential for this plugin are not loading properly.

These js files are missing and that’s the reason.
jquery.ui.core.min.js
jquery.ui.datepicker.min.js
bigContactForm.js
