[Plugin: Bit.ly Shortlinks] Get Shortlink Button Disappears

I have tried deactivating and reactivating the plugin several times. I have disabled all other plugins. But once the Bit.ly Shortlinks plugin is activated, the “Get Shortlink” button disappears and nothing shows in its spot. This only started happening with WordPress 3.4.1
