I’ve inserted <?php if (function_exists(‘get_cat_icon’)) get_cat_icon(); ?> into many hooks and still get nothing. Icons are assigned and displaying in the options panel.

One the icons are assigned, you need to paste the code :
<?php if (function_exists('get_cat_icon')) get_cat_icon(); ?>
at the right place in your template files. Did you use “Template Tags” panel as explained here ? It shows you where to paste the call to get_cat_icons().
Yeap, the template tags screen is empty because I pasted all the suggested places.
I think that’s because your theme does not use the “usual” way to display post titles (but I’m sure the widget’s working, though). The Template Tags Panel is not able to detect the location for all the themes. I’m sorry I can’t help you, because I don’t have the canvas theme : it’s not free…
I’m working on Category Icons Lite, and maybe it’ll do the job itself, without the need of the template tags. “Stay tuned”.
I’ve released the Category Icons Lite v1.1.4. Not available from WordPress UI yet, but it will be soon. You can try it, if you want.
I’ve tried the Lite version but where’s the option to upload the icons?
