Upon activation plugin causes an error 500 and crashes site until plugin is deactivated or deleted via FTP. I realize my WP version is far out of date, but figured you may want to know about the issue anyway.

@bytesize Media – Can you elaborate on how old we are talking? Also maybe some server details such as PHP version. This is not commons so likely something along those lines.
