Naturally, it takes longer to upload an image PLUS do optimization, so if your server timeout is configured too short (30 seconds or less) AND your server blocks background/async optimization, you will get http errors.

It’s a flaw in the design of the plugin, without the plugin it takes under 1 second to upload the image, with the plugin enabled it never happens.
I have a high-end web server, there’s no question the plugin is at fault because I’ve tested it thoroughly and get consistent results.
Doesn’t matter how “high-end” your web server is, it can be configured in such a way as to not allow optimization to work, and throw errors: https://docs.ewww.io/article/67-upload-http-error
