[Plugin: Contact Form 7] Adiitional tags in forms

I’m trying to create a form that asks for name, email and two other pieces of information:
<p>Your Name (required)
[text* your-name] </p>

<p>Your Email (required)
[email* your-email] </p>

<p>Subject
[Email List Subscription] </p>

<p>Your Message
[Please Subscribe me to the WNCC Email List. I understand my email will not be given away and further that I can unsubscribe at any time.]

[What was the Number of your Cursillo?]
[text* cursillo-number ]

[Where did you make your Cursillo?}
[text* cursillo-location ]</p>

Whenever I test this with data, I receive only the name, email and text message “Please subscribe…” none of the other appears.
Any tips?
