[Plugin: Contact Form 7] using HTML tables within the form

I would like to create a “form-table” which looks like this:

If I create a standard HTML table in the Contact 7 form box, and then include the standard fields, everything works.

However, as the mockup shows, I want to include checkboxes in the right column of the table. I’ve tried with no success.

I’ll create a separate thread to ask questions about the remaining data / charts I’d like to put into the form.

I’ve seen many websites with similar layouts, selling items or digital products. If anyone knows of another plugin (perhaps some kind of shopping cart) which might accommodate my needs, please let me know. Thanks much!
