I had the plugin installed and active with maintenance mode on, and when I logged on again it had crashed and removed itself from my plugin list. How could this happen?

    Please respond asap.

The plugin can’t remove itself from the list of plugins. It simply does not have the code to do that. Somebody removed it.

Is it possible for it to crash and switch the maintenance mode off on its on?

On it’s own no, but if something changes/crashes the database and alters the active plugins options key it can be disabled that way.
