[Plugin: Custom Contact Forms] 2nd value?

I’ve benn trying everything I can think of but I just don’t see how to add additional values to the dropdown feild. When I change the value and click attach nothing happens. It says “no feild options to attach” but all it has is a space for the initial value. Where do I add additional values?
