[Plugin: Custom Contact Forms] After upgrade doesn't work

I did upgrade to latest version and now always report that 1 mandatory field isn’t chosen and it IS.
It’s about radio button choice, I click whichever radio button but doesn’t work Form says you haven’t chose this field.

What to do now?
