[Plugin: Custom Contact Forms] cannot have success on submit

I created a form at http://coryon.com/fb/free-website-evaluation/

When I enter the data and submit, I don’t get any error messages or any success messages, it just goes to a page that says “return to form”.

I like this plugin if I can get it to work.
