    I was originally writing this post to detail a query error preventing me from creating a form. The source of the error was related to the Windows installation for MySQL as detailed here: http://bugs.mysql.com/bug.php?id=13794

    As a separate note, I would like to suggest that the author test his plugin with WP_DEBUG turned on so that he could address the various Notices from his plugin. As a developer, I rely on debug mode for my own development but the amount of messages coming from CCF make it difficult.
