[Plugin: Custom Contact Forms] Simply doesn't post a form.

    WP3.3.2:
    Added plugin, set up form, added code to Contact Us page (only content on the page).
    The Submit and Reset buttons appear, but no form content.
    Followed instructions to deactivate/activate: no better.
    Followed instructions to uninstall/install: no better.
    I work at least 10 hours a day on multiple sites, I don’t have time to piddle around with this; especially when I read in the news section that the developers were gone for 4 months. “Really?”
