[Plugin: Custom Permalinks] Where do I set the custom permalink?

I have installed this plugin on two separate sites, but I don’t see where inside of a post or page the permalink can be changed. I see on the plugin website that there should be a menu to edit it (see: http://atastypixel.com/blog/wp-content/uploads/2008/11/200811011939.jpg), but I do not see that on any of my pages or posts. At all. Is it not on a specific post or page?
Any help would be greatly appreciated!
