I’m not sure I understand your question.
On your blog page I see 3 posts with this image. I can see that is set as the DFI in your media settings.
Can you explain the problem?

I have a feed that feeds news to the blog and sometimes it pushes content with zero images hence I decided to use your plugin. Basically when the post gets posted and it has no image your plugin doesn’t automatically update it with the default image unless I deactivate the plugin – clear cache and reactivated. If you visit the site now you would notice that there is a post that has been posted with no image. In order to have the default image to show I have to deactivate – clear cache – activate.
I see there are 2 posts from today (10-dec) with broken images.
But they don’t use the DFI but https://www.globeboss.com/wp-content/uploads/2021/12/wp-header-logo-32.png
Which has errors.
What plugin do you use for caching?
This is a weird problem
like i said earlier, it only works when i deactivate – delete – reinstall – reactivate and insert image… as it is showing at the moment..

I’ve tested on a clean site with DFI an wp-sweep installed.
And everything I test works. I can’t get it to break.
But I do see several broken images.
The first 4 post on the blog have broken images. Like https://www.globeboss.com/wp-content/uploads/2021/12/wp-header-logo-69.png
But these 4 posts have a (broken) featured image. The five below have a working Default featured image.
Can you check the first 4 blog posts. I think these posts just have broken images.
