How are those images used in your website? What the links look like?
I have the same behaviour.

I have 3.503 items that occupy 3,5 GB it took me over 30 minutes to scan and at one time I got a time out and the other time it just stopped at 55%.
The system knows when they are part of the series within a post so that works fine too. But what I noticed is that when I navigate the images though the above link it shows all the images. Including the ones that aren’t in the post itself. No idea if that’s normal behaviour but that could explain why your plugin is not be able to tell which images are connected and actively used.
Other related question: Does your plugin also remove the other sizes of the original image that are created by wordpress?

We set the PRO version of Media Cleaner to scan the media library. The scan dosent complete and reported no unused items in the media library, which I know is incorrect.
